Last chance to have your say on Dementia services!!

 

Our survey closes today. Help to influence change by completing the survey and have your say.

Healthwatch Leicester, Healthwatch Leicestershire and Healthwatch Rutland are working together to undertake a study into dementia services and looking at people’s experiences of living with dementia (memory problems) and care services such as.

 Getting a diagnosis
– Being given good information about dementia
– Being informed about and accessing available support, such as:
– Financial support
– Carer Support
– Health and social care support
– End of life care

This questionnaire is about living with dementia, whether you are a person with dementia, or whether someone close to you has dementia (or had dementia in the past 5 years).
The questionnaire is anonymous and the answers you provide will be kept confidential.

They will be used to;

a) Help us to understand the experiences of living with dementia
b) What support services work well or not so well
c) Where there are gaps in services

We will publish a report to share with dementia service providers to recommend changes they might make.
