Share your experiences of Long Term Health Conditions

We are carrying out a project to understand the experiences of people with multiple health conditions in accessing and using health services; how they have been involved in decisions about their health care and how they would like to be involved in the future.

It is estimated that a quarter of the population of the UK are living with more than one long term condition, and this rises to two thirds of people aged 65 or over.

There is some evidence that care of conditions is still built around treating each condition on its own rather than treating the whole person.

Patients should be involved in their own care and planning that care and treatment, and this is even more important for those with more than one long term health condition.
