Hospital Discharge Survey
If you or someone you care for have been discharged from the hospital in the last two years, we would like to hear from you.
We are gathering feedback about the experience of being discharged from the hospital. If you or a loved one have left the hospital in the last two years, your feedback could help shape future services and ensure patients receive the best care.
The Hospital Discharge Survey aims to gather insight into what worked well and what needs improvement. From discharge planning to follow-up support, we want to hear about your journey.
Harsha Kotecha, Chair of Healthwatch Leicester and Healthwatch Leicestershire, said: “By sharing your experience, you can help improve care for others in our community. We encourage everyone who has been discharged from hospital to take a few minutes to complete our survey.”

The survey will close on Friday 23 May 2025, so please have your say if you have experience of hospital discharge.
All responses are anonymous, and the findings will be shared with local health and care services to drive improvements.
