We are gathering feedback about the experience of being discharged from the hospital. If you or a loved one have left the hospital in the last two years, your feedback could help shape future services and ensure patients receive the best care.
The Hospital Discharge Survey aims to gather insight into what worked well and what needs improvement. From discharge planning to follow-up support, we want to hear about your journey.
